Ready to Apply? Here's what you will need:

  • Your basic information, including your address and the last 4 digits of your social security number

  • Basic information about your telephone service and/or internet service, such as "Who is your service provider?" (who you pay your phone or internet bill to)

  • A Photo or a Scan of the following:

    • Driver's License​ / State ID / Identification Card 

      • Only one type of ID is needed - to show proof of residency

    • Your recent landline (home) phone bill - OR - internet bill

    • Benefits or Income Verification​ (click/tap to view options)
